OKRs should be one of the most powerful tools in product management. Objectives and Key Results, when done well, align entire organisations around outcomes instead of outputs. They force clarity on what success actually looks like. They create focus.
But most OKR implementations I've seen are a disaster. Not because OKRs are flawed — but because organisations fill the framework with the same output-focused thinking they were trying to escape.
Why Most OKR Implementations Fail
I've seen the same failure modes in dozens of organisations:
Failure 1: Output Key Results
The most common problem. A team writes an Objective like "Improve customer onboarding experience" and then creates Key Results like "Launch new onboarding wizard" and "Ship 3 tutorial videos." Those aren't key results — they're tasks. You could deliver all of them and onboarding could still be terrible.
An outcome-focused Key Result would be: "Increase Day-7 activation rate from 35% to 50%." Now you're measuring whether onboarding actually improved, and the team has freedom to figure out the best way to get there.
Failure 2: Too Many OKRs
I've seen teams with 8 Objectives and 30 Key Results for a single quarter. That's not focus — it's a wish list. If everything is a priority, nothing is. Three Objectives maximum. Two to three Key Results each. If it doesn't fit, it's not important enough this quarter.
Failure 3: No Cadence of Review
OKRs that are set in January and reviewed in March are just New Year's resolutions. Effective OKR implementations have weekly check-ins: Are we making progress? What's blocking us? Do we need to adjust our approach? The OKR becomes a living conversation, not a quarterly report card.
Failure 4: Cascading as Command-and-Control
Some organisations cascade OKRs top-down like a work breakdown structure. The CEO sets OKRs, VPs derive sub-OKRs, directors break those down further, and teams get told exactly what to achieve. This defeats the purpose. OKRs should align, not cascade. Teams should set their own OKRs in response to strategic direction, not receive them from above.
Output Key Results vs Outcome Key Results
This is the single most important distinction in OKR practice. Here's a side-by-side:
- Output KR: "Launch mobile app." Outcome KR: "20% of transactions happen on mobile."
- Output KR: "Ship search redesign." Outcome KR: "Reduce search-to-purchase time from 4 minutes to 90 seconds."
- Output KR: "Complete API documentation." Outcome KR: "Reduce developer integration time from 2 weeks to 3 days."
Notice the pattern? Output KRs describe what you'll build. Outcome KRs describe what will change for users. The difference isn't semantic — it fundamentally changes how teams work. With outcome KRs, teams experiment, learn, and iterate toward the target. With output KRs, they just build what's on the list.
How AI Can Help Validate OKRs
This is where it gets interesting. AI can serve as an OKR quality coach. Feed your draft OKRs to an AI and ask it to identify output-disguised-as-outcome key results, challenge assumptions about measurability, suggest alternative metrics, and stress-test whether achieving the key results would actually achieve the objective.
AI can also help with baseline discovery. "What's our current Day-7 activation rate?" might require pulling data from multiple systems. AI assistants can help gather and synthesise this data so teams set realistic, evidence-based targets instead of aspirational guesses.
The best use of AI in goal-setting isn't to write the goals — it's to challenge them. A good AI coach asks the questions that people are too polite to ask in a room full of stakeholders.
Practical Tips for Getting Started
- Start with one team. Don't roll OKRs out across the entire organisation at once. Pick one product team, help them write genuinely outcome-focused OKRs, and learn from the experience.
- Ban output key results. Use the test: "Could we achieve this KR and still fail to achieve the Objective?" If yes, it's an output KR.
- Establish baselines before setting targets. You can't aim for a 20% improvement if you don't know where you're starting.
- Review weekly, adjust monthly, reset quarterly. OKRs are a thinking tool, not a compliance tool.
- Celebrate learning, not just achievement. An OKR scored at 0.3 that taught you something valuable about your customers is worth more than a 1.0 that just confirmed what you already knew.
OKRs work when they shift the conversation from "what are we building?" to "what change are we creating?" That shift — from outputs to outcomes — is one of the hardest and most important changes a product organisation can make.
